How To Fill Out Your Prescription

Single Vision Prescription

If your prescription looks like the example below, simply select the Single Vision option and enter the required information accordingly.

Single Vision Prescription with Prism

If your prescription includes prism correction, make sure to enter the prism values in addition to the other standard prescription fields.

Progressive Prescription

If your prescription is for progressive lenses, please refer to the examples below for how to enter your details correctly.

Sample 1:Enter values as shown.
Sample 2:Convert numbers to decimal format (e.g., +325 ➝ +3.25, -250 ➝ -2.50).
Sample 3:Use the difference between near and distance vision to calculate your ADD value.

For example:
Near OD -0.50 – Distance OD -2.00 = ADD +1.50
Note:All three prescriptions result in the same lens prescription.

Reading Prescription

If your prescription is for reading glasses, please enter the information as shown in the example.

Other Information You Might Need

Here are a few terms you may see on your prescription and what they mean:
OD: Oculus Dexter – Right Eye
OS: Oculus Sinister – Left Eye
PL (Plano): No spherical correction needed, enter 0
DS (Diopter Sphere): No astigmatism correction for this eye
Please make sure to enter all values as shown in the example. Again, if your prescription doesn't list a PD, you'll need to provide it.
